Google: AMIE Reached Primary Care Physician Level in Nature Study on Disease Management

Google published a study in Nature in which its medical AI AMIE matched the level of primary care physicians in disease management — one of the most significant clinical AI results to date.

Blinded Comparison with 21 Physicians

The study was blinded and compared AMIE with 21 primary care physicians using standardised patients — trained actors simulating clinical cases. AMIE matched physicians in overall disease management reasoning and was statistically superior in two measures: treatment plan precision and alignment with clinical guidelines. The results were published in Nature with DOI reference s41586-026-10764-5.

Dual-Agent Architecture

AMIE is a medical AI system built from two agents. The first is an empathetic dialogue agent that conducts the conversation with the patient, while the second is a deep disease management reasoning agent that cross-checks hundreds of pages of clinical guidelines before proposing a plan. This separation of patient communication from complex medical reasoning likely contributed to the advantage in plan precision.

What Comes After the Nature Study?

Following publication, Google launched a national randomised study in real virtual clinics, taking the test out of controlled conditions with standardised patients and into actual clinical practice. A randomised study is the gold standard of medical evidence, as it randomly assigns patients to isolate the effect. This will determine whether AMIE’s advantage in plan precision holds up outside laboratory conditions.
